Description
This position is responsible for coordination and provision of facilitating enrollments, transfers, and discharges for persons with Intellectual and Developmental Disabilities into and out of community programs, Nursing Facility Diversions/Transitions, Medicaid waiver programs, ICF-ID, and State Supported Living Center services. This position will also provide Enhanced Community Coordination to qualifying individuals. Work involves developing and maintaining both short and long term contact with clients, clients’ families, service providers, and community agencies. Position will complete the PASRR process upon timeliness for contract requirements. This position requires the ability to identify service needs, work under pressure and stressful situations in a professional manner, be thorough in assessment and documentation, identify a variety of community resources to meet the service needs of each individual, negotiate and advocate with the service units for each individual, and develop a positive relationship with both the Nursing Facility staff, the individuals served, LAR’s, and service providers. These duties are performed under supervision and in accordance with established policies with some latitude for independent judgment. The position represents the Center as a liaison, and informs agencies, consumers, and families of services and supports. This positions requires the ability to work well with a team of staff, however much of the work is completed independently and with significant latitude for the use of initiative and independent judgment. Professionalism is mandatory.

Qualifications
•Requires a Bachelor’s Degree in a social services related field or meet credentialing requirements for IDD Service Coordination.
•Must have at least two years of experience working directly with individuals with intellectual or other developmental disabilities.
•Must be familiar and competent in using standard office software (Excel, Word, etc…).
•Must have a current Texas Driver’s License and qualify for Center insurance coverage.
