Description
The Care Coordinator LVN is committed to improving health outcomes and the quality of life for chronically ill and high risk individuals served at Betty Hardwick Center. This positions works with an interdisciplinary team to coordinate the medical, psychosocial, emotional, therapeutic, and recovery support needs of individuals. The Care Coordinator LVN will work with behavioral health and medical health providers to share information to assist in obtaining effective treatment of the whole person for their overall wellbeing. This position interacts directly with individuals in services and works as a part of an interdisciplinary team. Responsible for linking individuals to the appropriate behavioral and medical health services, and educating clients on physical/mental health well-being.

Qualifications
LVN licensed by the State of Texas.
-Experience in mental health and/or medical services for children and adults is preferable.
-Experience with care coordination or case management is a plus.
-Must demonstrate leadership and communication skills, possess excellent organization skills with ability to keep accurate records, and have experience with health information systems and strong computer skills.
-Requires flexible hours, involves extensive paperwork, and will require self-directed performance.
-Travel and the use of a personal vehicle may be necessary.
-Must have a valid Texas driver’s license and be an insurable driver through the Center’s insurance carrier.
